Yeah.. I once strapped a socket 7 cpu heatsink onto a 3DFX Voodoo 3... took up the AGP slot and 2 PCI slots below it.
Here's a pic of another Voodoo that I strapped a 6cm fan to..

If you look close at the pics, I pressed some 3mm locknuts into the corner holes on the fan and then ran 2 long 3mm screws through the back of the card up through the heatsink mounting holes and into the locknuts on the fan.

Man.. that's an old pic.. pre-married Serzoni.
No more long hair (you can see a hint of it.. it was pulled back in the pic), 50 lbs heavier, and a much faster video card now.
And for the record- I have "regular" sized nipples.. and they're very hairy despite my chest looking like a barren wasteland..lol

I used heatsink screws that many older heatsinks had. They are basically a short fat wood screw that fits in between the heatsink fins (but not too tightly)

yeah.. definately replace the old paste. The heatsink should come with a small tube or pouch of it. If not, a tube of it is available at radioshack for about $2

Ya, I was amazed at difference of thermal pastes too. While experimenting with water cooling and such I got tube of Artic Silver and gained another 2-3 degrees.
Little bit more but worth it.
Just add water to your system.
